Transcript Request

All transcripts must be requested in person or in writing.

1. Provide the requested information and mail the form to Culinary School of the Rockies. Please include:
   Full name
   Former name (if applicable)
   Social Security Number (SS#)
   Dates of Attendance or Month and Year of Graduation
   Address(es) to which the transcripts are to be mailed
   Your signature
   Fee per copy payable by check, or money order



2. Mail your request to:

3. Services and Fees: $5.00 per copy. Processing time will take 3 to 5 business days.
